The day you realize you or your loved one needs specialized care is a difficult one. The time, energy, and emotions that go into researching care options, planning out your budget, and coming to terms with a new reality are too overwhelming to experience alone. That’s why Senior Solutions’ Transition Services promises that we will be with you throughout the entire process, making sure that you feel confident in your decisions and comfortable with your budget.

Why We’re Different

1. We’re local.

We have been helping Seniors in Middle Tennessee since 2010, and we feel confident that we know this area and its available Assisted Living
Communities better than anyone.

2. We’re invested.

Unlike most Senior ‘Placement’ Services, we will go above and beyond. We want to get to know your unique situation and help you transition into an environment that feels comfortable for you.

3. We’re consistent.

We’d like you to think of us as an extension of your family because we genuinely care about your future. Though we will always make sure you are kept in the loop, your consultant will handle communication and scheduling between communities, giving you the time and peace of mind to prepare for your transition.

What We Do

Let us help ease you into your lifestyle with these three easy steps.

1. Meet

A consultant will schedule an initial, brief evaluation with you over the phone. During this exchange, we will discuss your unique situation and budget to determine the extent of your needs, allowing us to better understand which plan is right for you.

You will also schedule an in-person meeting.

2. Plan

After the initial phone call, you will meet with your consultant in person to discuss your options in detail. We will map out your best avenue of care as it applies to your abilities and your budget. We will also discuss further resources that can help save you money, should you qualify.

3. Choose

Depending on which avenue of care suits you best, we will help you choose the right communities and services to utilize during your transition to assisted living. Together, we will tour communities, meet with staff members, and make decisions regarding your future care.

What do you mean by “Avenues of Care”?

Though each and every person inevitably reaches a point where they need specialized care eventually, it is a different and unique journey for everyone. Just because one route of care worked for a friend or relative, does not mean it is the right path for you.

Independent Living

Independent Living is the best option for seniors looking for an extra set of hands. Perhaps you or your loved one needs help with housekeeping, laundry, transportation, or cooking, but can still, for the most part, continue living life independently.

Assisted Living

Assisted Living is the most popular choice amongst seniors and their loved ones. It takes Independent Living one step further by including full-time medical and
custodial care. However, seniors who choose Assisted Living can utilize the available resources as much or as little as needed.

Long-Term Care

Long-Term Care is designed for seniors who need more in-depth care. Those who suffer from Alzheimer’s, dementia, or other ailments that require round-the-clock care are most suited for this avenue.

What if I’m not ready to leave my home?

The answer to this question is precisely why we make a point to become very familiar with your needs and budget before making any decisions regarding your future. Oftentimes, seniors and their loved ones are misinformed about the options that are available to them and feel forced to leave their homes prematurely. With Senior Solutions Transition Services, this is never the case. We assure you that all of your options will be communicated to you. For clients who wish to stay at home, we can make that happen, as long as your health allows it.
