Our care management teams work with caregivers to ensure the proper plan of care is followed for your loved one. While making home visits at least once a month, care managers act as an additional expert resource when determining your loved one’s changing needs. They will work with the caregiver and the client’s family to adjust the client’s care plan. If necessary, the care manager and family members can provide additional training for the caregiver to address these changes.
